HelpAge Tanzania (HAT) is inviting applications for the position of Integrated Care Nurse (ICN) based in Kigoma Region. This opportunity is ideal for qualified nursing professionals who are passionate about delivering person-centred healthcare and improving the wellbeing of vulnerable populations within humanitarian settings. The successful candidate will provide integrated care services to older people, persons with disabilities, individuals with chronic health conditions, and other people with specific needs in Nyarugusu refugee camps.
Working closely with health facilities, protection teams, community structures, and humanitarian partners, the Integrated Care Nurse will ensure coordinated, continuous, and high-quality care. This role offers an opportunity to contribute to HelpAge Tanzania’s mission of promoting dignity, inclusion, and equal opportunities for older persons and people with disabilities across Tanzania.
Responsibilities
- Deliver person-centred and holistic healthcare services to vulnerable populations.
- Provide preventive, curative, rehabilitative, supportive, and end-of-life care where appropriate.
- Support older people, persons with disabilities, individuals with chronic illnesses, unaccompanied and separated children, and other people with specific needs.
- Coordinate care with health facilities, protection teams, community structures, and humanitarian partners.
- Promote continuity of care and improve clients’ quality of life, functional ability, comfort, dignity, and psychosocial wellbeing.
- Participate in multidisciplinary case management and referral processes.
- Ensure healthcare services are delivered in accordance with humanitarian principles and professional standards.
- Maintain accurate patient records and prepare required reports.
- Uphold safeguarding, protection, disability inclusion, and gender equality principles in all aspects of service delivery.
- Contribute to community-based health interventions and awareness activities as required.
Qualifications Required
- Bachelor’s degree in Nursing or another relevant health-related field.
- Valid professional registration for nursing or the relevant health profession.
- Relevant professional experience in humanitarian and/or development programmes.
- Experience working with non-governmental organizations (NGOs), international NGOs (INGOs), or community-based organizations.
- Strong understanding of gender equality, disability inclusion, safeguarding, and protection principles.
- Ability to work independently and collaboratively within multidisciplinary teams.
- Excellent communication skills in both English and Kiswahili.
- High level of integrity and commitment to HelpAge Tanzania’s mission and values.
